Job summary

Beth Israel Lahey Health is seeking an Advanced Practice Provider (NP or PA) to join our interdisciplinary palliative care team in outpatient settings across Burlington and Peabody, MA, with potential inpatient coverage as needed.

The APP will conduct comprehensive assessments, manage symptoms, lead advance care planning conversations, and coordinate care with oncology, primary care, nursing, and social work to support seriously ill adults and their families.

Qualifications

• Provide comprehensive palliative care assessment for adults with serious, life-limiting illnesses.
• Lead advanced care planning discussions and goals-of-care conversations with patients and families.
• Coordinate care with interdisciplinary teams to align treatment with patient values and preferences.

Responsibilities

• Outpatient palliative care in Burlington and Peabody with potential inpatient coverage.
• Educate patients and families on symptom management, prognosis, and care options.
• Document clinical care and participate in quality improvement initiatives.
