River Valley Rehabilitation & Healthcare Center in Del Norte, CO is hiring a compassionate Occupational Therapist (OT) to join our rehabilitation team. You will evaluate residents, develop individualized therapy plans, and provide skilled OT services in a SNF/LTC setting.

Qualifications

• Licensed OT in Colorado or eligible for licensure.
• Graduate of an accredited OT program.

• Evaluate residents and develop OT treatment plans.
• Deliver skilled OT services in SNF/LTC settings.
• Document evaluations, treatments, progress notes, and discharges.
• Collaborate with physicians, nursing, and care teams.
• Educate residents and families on goals and adaptive techniques.
• Promote independence, safety and quality of life through evidence-based therapy.
