Job summary

River Valley Rehabilitation & Healthcare Center in Del Norte, CO is seeking a compassionate Occupational Therapist (OT) to join our growing rehab team and help residents regain independence in a skilled nursing setting. You will evaluate residents, develop individualized treatment plans, document progress, and collaborate with physicians and nursing staff to maximize outcomes while supporting families.

Qualifications

• Current Occupational Therapist (OT) license in Colorado or eligibility for Colorado licensure.
• Graduate of an accredited Occupational Therapy program.
• SNF or LTC experience preferred, but new graduates are encouraged to apply.
• Strong communication, clinical, and documentation skills.
• Passion for delivering compassionate, resident-centered care.

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans.
• Provide skilled occupational therapy services in a SNF/LTC setting.
• Document evaluations, treatments, progress notes, and discharge summaries.
• Collaborate with physicians, nursing staff, and the interdisciplinary care team.
• Educate residents, families, and caregivers on therapy goals and adaptive techniques.
• Promote resident independence, safety, and quality of life through evidence-based therapy.
• Follow all state and federal regulations.
• Adhere to facility policies and procedures.
• Perform other duties as assigned.

Occupational Therapist (OT) Qualifications

• Current Occupational Therapist (OT) license in Colorado or eligibility for Colorado licensure
