Job summary

The Cancer & Hematology Centers are seeking a Nurse Navigator with oncology expertise to coordinate patient care across the cancer continuum. You will educate patients, guide them through diagnosis, treatment options and appointments, and help navigate financial resources and support services.

This full-time role is based at the Kenmoor clinic, operating 8:00am–6:00pm in a schedule of three 10-hour shifts Mon–Fri, offering dedicated multidisciplinary collaboration and a focus on timely,

Qualifications

• Licensed RN in Michigan with oncology focus
• Oncology-specific certifications or willingness to obtain within set timelines
• Experience with ambulatory oncology workflows and care coordination

Responsibilities

• Serve as primary clinical contact for patients, families and caregivers.
• Coordinate diagnostic workups, treatment starts and follow-up visits across ambulatory oncology services.
• Collaborate with physicians, NPs, infusion nurses, pharmacists, social workers, dietitians and financial counselors.
• Educate patients about diagnosis, treatment plan, and symptom management.
• Support prior authorizations, referrals and test coordination to avoid delays.
• Address barriers to treatment adherence (transportation, finances, scheduling, language).
• Track patient progress and document navigation interventions in EHR.
• Perform telephone triage for urgent symptoms and escalate as needed.
• Coordinate follow-up and care management for high-risk patients.
• Partner with Nursing Leadership for quality improvement and workflow optimization.
