Job summary

Crossbridge Hospice in Cincinnati, OH is seeking an On-Call RN Case Manager (PT) to provide end-of-life care in patients' homes or community settings. The role includes intermittent skilled nursing support and supervision of hospice aides.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Valid driver license and proof of auto insurance.
• Two years of RN experience.
• Two years of Hospice/Palliative Nursing experience preferred.

Responsibilities

• Assume primary responsibility for the patient/family caseload during on-call hours.
• Coordinate care and collaborate with triage and Branch RN Case Managers.
• Maintain and complete appropriate care coordination notes for visit notes and other pertinent clinical information.
• Assist, provide, or arrange for delivery of medications and supplies after business hours, weekends, and/or holidays.
• Attend interdisciplinary team meetings during on-call periods for team discussion and evaluation of patient/family needs.
• Admit patients eligible for hospice services under physician orders with the triage team.
• Report patient status to clinical leadership and attending physicians.
• Develop patient care plans addressing identified problems and mutually agreed goals with patients.
• Apply the 4Ms framework (What Matters, Medications, Mentation, Mobility) and provide Age-Friendly Care.
