Job summary

Covenant Living is seeking an occupational therapist contracted to provide home health services, focusing on patient assessment, treatment planning, and coordination with the care team.

Requires a Bachelor’s in Occupational Therapy, current OT license, and CPR certification, with 2+ years of experience. Competitive hourly pay and comprehensive benefits are offered. Positions support residents across Covenant Living communities and CovenantCare at Home.

Qualifications

• Required Degree: Bachelor’s Degree in Occupational Therapy from an accredited institution.
• Preferred Degree: Master’s Degree in Occupational Therapy from an accredited institution.
• Certificate(s): OT License and CPR certification, if required by state.

Responsibilities

• Evaluates patients’ functional status and home environment to identify hazards and barriers.
• Develops treatment plans and establishes goals for improved function.
• Communicates plan of care to referring physician and other professionals.
• May teach new skills or retrain patients in activities of daily living.
• Maintains clinical records and coordinates with other care team members.
