Job summary

Angels Care Home Health seeks an Occupational Therapist to assess patients, identify needs and determine the appropriateness of care plans. You will document, revise, and complete the Plan of Care and progress notes for ongoing patient care.

Coordinate with team members, apply infection control and universal precautions, and ensure quality care across the patient’s environment. OK license and 2 years of OT experience required.

Qualifications

• Current state license as an Occupational Therapist.
• Two years of experience as an Occupational Therapist.
• Current CPR and Hepatitis profile.
• Knowledge of durable medical equipment.

Responsibilities

• Perform clinical assessment; identify patient needs and appropriateness of patient.
• Document, prepare, and complete a Plan of Care, progress notes, and other clinical record documentation on an ongoing basis.
• Revise the POC based on evaluation and ongoing assessment.
• Applies infection control and universal precautions in coordinating/performing patient care.
• Uses effective interpersonal relations and communication skills with the team to achieve outcomes.
