Job summary

Sky Heart Home Care Services in Apex, NC is seeking a compassionate Care Transport Associate to join our Non-Emergency Medical Transportation team. You will transport clients to medical appointments, assist with boarding and mobility devices, and ensure safe, on-time arrivals.

You will perform pre-trip inspections, maintain trip logs, communicate professionally with clients and staff, and uphold HIPAA, dignity, and confidentiality in every ride.

Qualifications

• Valid driver's license with a clean driving record.
• Experience in NEMT, medical transport, or patient care preferred.
• CPR and First Aid certification (or willingness to obtain).
• Ability to assist clients with mobility challenges, including wheelchair securement.
• Strong punctuality, time management, and organizational skills.
• Compassionate, patient, and professional demeanor when serving elderly or disabled individuals.
• Ability to pass a background check and drug screening.

Responsibilities

• Safely transport clients to and from non-emergency medical appointments, therapy sessions, and healthcare facilities
• Assist passengers with boarding, exiting, and securing wheelchairs, walkers, and other mobility devices
• Maintain accurate trip logs, mileage records, and documentation in compliance with NEMT regulations
• Communicate professionally with clients, caregivers, dispatchers, and medical staff
• Conduct pre-trip vehicle inspections to ensure safety, cleanliness, and proper equipment function
• Follow scheduled routes while adapting to real-time changes or urgent transport needs
• Uphold patient dignity, confidentiality, and HIPAA compliance at all times
