Job summary

CenterWell Home Health seeks a Medical Social Worker to evaluate and develop personalized care plans addressing emotional and social needs. The role includes supervising the Social Worker Assistant and ensuring compliance with company policies and regulations.

Responsibilities cover assessing social/emotional states, planning care with the team, coordinating resources, and participating in discharge planning. Requires MSW/LIC, CPR, English fluency, and willingness to travel locally.

Qualifications

• Master's or doctoral degree from a CSWE-accredited social work program.
• Social Work licensure in the state of practice (required by state law).
• Valid driver's license with auto insurance and reliable transportation.
• CPR certification.
• Minimum of one year of social work experience in health care, home health, or hospice.
• Knowledge of discharge planning and community resources.
• Excellent oral and written communication and interpersonal skills.
• Fluent English (read, write, speak).
• Ability to organize tasks, develop action plans, and handle stressful situations.
• Willingness to travel locally.

Responsibilities

• Assesses the patient’s social and emotional state related to illness or injury and care needs.
• Evaluates relationships of medical/nursing needs in the home setting and resources.
• Obtains community resources to assist in resolving issues hindering recovery.
• Instructs patients/families in coping with social/emotional responses; ongoing assessment.
• Assists physicians and health team in understanding social/emotional factors; contribute to Plan of Care.
• Observes and reports changes; participates in discharge planning.
• Participates in interdisciplinary care team for care coordination.
• Supervises Social Work Assistant to ensure compliant services.
• Maintains and submits documentation per company and regulatory standards.
