Job summary

Green Hills Center for Rehabilitation and Healing is seeking a compassionate Social Worker to join our interdisciplinary team. In this role, you will address psychosocial needs, facilitate care planning, and support residents and families within a skilled nursing environment.

Responsibilities include discharge planning, coordination with nursing and therapy teams, advocacy for residents’ rights, and linkage to community resources while maintaining regulatory-compliant documentation.

Qualifications

• Bachelor’s Degree in Social Work (BSW) required; MSW preferred.
• Experience in a skilled nursing facility or healthcare setting is a plus.
• Strong understanding of discharge planning and care coordination.
• Excellent communication and interpersonal skills.

Responsibilities

• Provide emotional support and counseling to residents and their families.
• Conduct psychosocial assessments and develop individualized care plans.
• Coordinate discharge planning to ensure smooth transitions of care.
• Advocate for residents’ rights and overall well-being.
• Collaborate with nursing, therapy, and medical teams to support holistic care.
• Assist residents and families with community resources and services.
• Maintain accurate and timely documentation in accordance with regulatory standards.
• Participate in care plan meetings and interdisciplinary team discussions.
