The LVN/LPN will provide direct patient care, document progress, and coordinate with the home health team. CPR certification, a valid LPN license, and at least one year of LVN/LPN experience are expected.

Qualifikationen

• Graduated from an accredited practical or vocational nursing program.
• Licensed Practical Nurse (LPN) in the state of operation.
• At least 1 year of LVN/LPN experience.
• CPR certification required while employed in a clinical role.
• Must be a licensed driver and able to travel to all business locations.

Aufgaben

• Performs technical nursing functions and direct patient care per the plan of care under RN supervision.
• Promotes optimal client care under RN supervision.
• Maintains a safe patient environment.
• Identifies significant abnormalities in patient data and communicates with the case manager and home health team.
• Administers prescribed medications and treatments; reviews all medicines for safety and interactions.
• Re-evaluates nursing needs and reports findings to the Case Manager.
• Counsels and instructs patients and families on nursing needs.
• Prepares clinical and progress notes; submits documentation per policy.
• Documents in patient charts in line with the nursing care plan.
• Communicates with home health team and coordinates services.
