Job summary

AltaMed Health Services is seeking a Licensed Vocational Nurse to develop and implement homecare services for program participants in California. You will coordinate all contracted home services, durable medical equipment, and nursing home care, and attend scheduled IDT and coordination meetings.

The role requires a CA LVN license, at least 1 year of LVN experience, a valid CA driver’s license, and a minimum BLS certification; experience with frail or elderly populations is highly desirable.

Qualifications

• Current LVN license issued by the State of California.
• Minimum one year of LVN practice.
• Experience with frail or elderly populations is preferred.

Responsibilities

• Develop and implement homecare services for participants.
• Coordinate all contracted home services, DME, and nursing home care.
• Attend scheduled IDT and coordination meetings.
