Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Current California LPN/LVN license and CPR certification.
• Minimum 3 years nursing experience; home health or medical/surgical preferred.
• Valid California driver's license with vehicle in good working order.

Responsabilités

• Provides direct patient care under the supervision of a registered nurse.
• Implements plan of care and documents accurately and timely.
• Assesses patient and educates family/caregiver on self-care and diagnosis.
• Coordinates home health services with other disciplines (HHA, OT, PT, MSW, ST, Dietician).
• Adheres to aseptic technique with equipment and supplies.
• Performs other duties as assigned by the registered nurse.
