Life Care Home Health is seeking a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) to provide skilled nursing care in patients' homes under the supervision of a registered nurse. You will coordinate with the interdisciplinary team, patients, and families to deliver high-quality care and ensure the Plan of Care is followed.
Ideal candidates have 1 year of LVN/LPN experience (home health preferred), a valid license in good standing, CPR, and reliable transportation.
Qualifications
- 1 year LVN/LPN experience in a clinical setting
- Home health experience preferred
- Active LVN/LPN license in good standing in applicable states
- Current CPR certificate
- Valid driver’s license and reliable transportation
Responsabilités
- Administer nursing care under RN supervision in patients' homes
- Coordinate care with interdisciplinary team and families
- Document timely and accurate information to ensure policy compliance
- Participate in case conferences and team meetings
- Promote company mission and maintain professional growth
- Follow infection control measures and OSHA standards
