Caringhand Home Health Solutions, Inc. is hiring a Licensed Vocational Nurse (LVN) to provide in-home nursing care under RN supervision in California.

This field-based, per-diem role requires travel to patients’ homes and involves implementing care plans, documenting care, and educating patients and families. Ideal candidates hold an active CA LVN license, CPR certification, and at least three years of nursing experience, including home health.

Qualifications

• Graduate of an accredited practical nursing or vocational nursing program.
• Current, active California LVN license.
• Minimum three years nursing experience, incl. home health or medical-surgical setting.
• Current CPR certification from American Heart Association.
• Strong communication and organizational skills.
• Valid driver’s license and reliable vehicle; ability to travel to patient homes.

Responsibilities

• Provide direct patient care within CA Nurse Practice Act and RN supervision.
• Implement the plan of care established by the RN.
• Accurate documentation aligned with the plan of care.
• Educate patients and caregivers; assist with procedures and treatments.
• Coordinate with RN/Clinical Supervisor to identify additional needs.
