Job summary

MCHWC Organization seeks a compassionate Licensed Vocational Nurse (LVN) in California to provide direct nursing care under physician supervision. You will monitor patients, administer medications, document in EHR, and participate in care planning with a multidisciplinary team.

The role requires a valid CA LVN license, CPR/BLS certification, strong communication, and familiarity with long-term care or outpatient settings.

Qualifications

• Current and valid LVN license in the state of California.
• CPR/BLS certification.
• Experience in a clinical, long-term care, or outpatient setting preferred.

Responsibilities

• Provide basic nursing care and administer medications and treatments as prescribed.
• Process medication refills per protocols under provider direction.
• Monitor and document vital signs and responses to treatment.
• Assist with patient assessments and report changes to the provider team promptly.
• Maintain accurate medical records per facility policies and regulations.
• Support patient education and health promotion.
• Coordinate with multidisciplinary team for continuity of care.
• Maintain a safe, clean environment; follow infection control protocols.
• Participate in staff meetings and quality improvement initiatives.
