Job summary

Fairfield Memorial Hospital Association in Illinois is seeking a licensed Practical Nurse (LPN) to join our Skilled Care Unit. Under RN supervision, you will provide direct resident care, supervise CNAs, and help ensure continuity of care for long-term residents.

The ideal candidate holds IL LPN licensure, BLS, and has experience in long-term or geriatric care. This PRN position offers competitive compensation with shift differentials and a supportive team dedicated to resident-centered care.

Qualifications

• Current Licensed Practical Nurse (LPN) licensure in Illinois; BLS certification.
• Long-term care, skilled nursing, or geriatric care experience preferred.
• Experience with electronic health records in skilled nursing setting is a plus.

Responsibilities

• Provide direct nursing care to residents per physician orders and care plans.
• Perform resident assessments and document findings in EHR.
• Administer medications and treatments per nursing standards.
• Monitor resident status and communicate changes to physicians and RNs.
• Supervise CNAs and support staff to ensure quality care.
• Maintain infection prevention protocols and compliance with policies.
