The successful candidate will have an active NH LPN license, CPR/BLS certification, and strong clinical judgment with excellent communication and multitasking abilities.

Qualifications

• Current, unencumbered LPN license in New Hampshire.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced setting.
• Strong clinical judgment, communication, and attention to detail.

Responsibilities

• Provide patient care per the written plan of care and physician's plan.
• Follow and implement individualized care plans developed by the Nurse Manager.
• Perform medication administration and treatments per the plan and policies.
• Monitor residents' conditions and report changes promptly to the supervising clinician.
• Maintain accurate documentation in line with orders, care plans, and standards.
• Support a safe, respectful environment for residents and families.
