Qualifications

• Current, unencumbered Washington State LPN license.
• Minimum one year of nursing experience.
• Assisted Living, long-term care, memory care, or geriatric experience preferred.
• Strong assessment, documentation, and communication skills.
• Ability to work independently while collaborating with a team.
• Genuine passion for serving older adults.

Responsibilities

• Monitor resident health and respond to changes in condition.
• Complete assessments and participate in care planning.
• Coordinate care with physicians, pharmacies, therapists, and families.
• Administer medications and oversee medication management.
• Support and mentor care staff.
• Assist with admissions, transfers, and discharges.
• Investigate and document incidents, falls, and resident concerns.
• Ensure compliance with state regulations and community policies.
• Promote a safe, comfortable, and engaging environment for residents.
