Qualifications

• Current, unencumbered Washington State LPN license.
• Minimum one year of nursing experience.
• Assisted Living/long-term care/geriatric experience preferred.
• Strong assessment, documentation, and communication skills; able to work independently with a team.

Responsibilities

• Monitor resident health and respond to changes.
• Complete assessments and participate in care planning.
• Coordinate care with physicians, pharmacies, therapists, and families.
• Administer medications and oversee med management.
• Support and mentor care staff.
• Assist with admissions, transfers, and discharges.
• Investigate incidents, falls, and resident concerns.
• Ensure compliance with state regulations and community policies.
• Promote a safe, engaging environment for residents.
