Job summary

Regency Park of Jefferson Nursing and Rehab is seeking a licensed practical nurse (LPN) to deliver high-quality resident care under RN supervision in a Campbell Street community.

You will participate in plans of care, perform focused assessments, document outcomes, and support a safe, respectful, and compliant care environment. Strong communication, leadership, and compassion are essential.

Qualifications

• Completion of a professional nursing education program and active LPN license.
• Current CPR certification required.
• Experience in skilled nursing preferred.

Responsibilities

• Plan and update resident care plans with RN input.
• Conduct focused assessments and compare data to health status.
• Communicate resident needs to primary caregivers and RN.
• Provide the highest quality nursing care per BON standards.
• Monitor and document interventions and patient responses.
• Ensure safety, dignity, privacy, and HIPAA compliance.
