Job summary

Eunice LA PDN PCS, a part of the LHC Group family, is seeking an licensed practical nurse to provide direct nursing care in patients’ homes within the service area. You will document visits and coordinate care under supervision of a registered nurse or supervisor.

Required qualifications include an active LA LPN license, CPR, and a valid driver’s license with access to reliable transportation. Home care experience is appreciated, with flexibility to meet patient needs.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current CPR certification.
• Current driver's license and access to dependable vehicle or reliable public transportation.
• Ability to function in diverse home environments regardless of client demographics.

Responsibilities

• Provides high quality clinical nursing services within the scope of practice, per plan of care, and in coordination with the health care team.
• Documents patient visits thoroughly in the electronic medical record.
• Participates in care coordination including reporting changes in condition and communicating with patient, family, physician and case manager.
