Optum and its Eunice-based provider Eunice LA PCS seek an LPN to deliver direct nursing care to acutely or chronically ill patients at home, under RN supervision. You will document visits and coordinate services to ensure high-quality care in your geographic area.

Requires current LA LPN license, valid driver's license or reliable transport, and at least 1 year of LPN experience. CPR certification is preferred, and flexibility to meet patient needs is essential.

Qualifications

• Current and unrestricted LPN licensure in LA.
• Valid driver's license and vehicle insurance, access to dependable transportation.

Responsabilités

• Provide high quality clinical nursing services within the scope of practice and in coordination with the health care team.
• Document patient visits thoroughly in the EMR per regulatory and payer requirements.
• Coordinate care and actively report patient needs and changes in condition to the team.
• Adapt to home environments and ensure care is culturally sensitive and patient-centered.
