Job summary

UnitedHealth Group, through Eunice Community Home Health Pediatri in Louisiana, is seeking a Licensed Practical Nurse to deliver direct nursing care in patients’ homes. You will document visits, coordinate with the health care team, and ensure high-quality care under supervision while complying with state practice standards.

The role requires LA LPN licensure, a valid driver’s license, and reliable transportation.

Qualifications

• Current and unrestricted Licensed Practical Nurse licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Provides high quality clinical services within the scope of practice, as defined by state laws and plan of care.
• Documents patient visits thoroughly and completely in the electronic medical record.
• Participates in care coordination, reporting changes in condition to the team and coordinating with case coordinators.
• Able to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
