Job summary

Qualifications

Responsibilities

• Monitor patient health and administer basic care within the LPN/LVN scope.
• Collaborate to meet patients' biopsychosocial and cultural needs.
• Provide for patient comfort and implement individualized care plans.
• Report status changes to RNs and physicians.
• Maintain accurate, thorough reporting and documentation of patient progress.
