The LPN will provide high-quality nursing care under RNs and physicians, administer medications, monitor health status, and collaborate with the healthcare team to implement care plans.

Qualifications

• Graduate of an accredited LPN program.
• Active LPN license.
• Experience in healthcare or long-term care preferred.

Responsibilities

• Provide basic nursing care and administer medications.
• Monitor vital signs and report changes to RNs/physicians.
• Assist in development and implementation of individualized care plans.
• Document health status and care provided in medical records.
• Communicate with patients and families about treatments and care plans.
• Assist RNs/physicians during procedures and dressings.
• Support residents with mobility, ADLs, and comfort.
• Ensure safety, PPE compliance, and infection control.
