You will administer medications, monitor health status, document changes, and collaborate with the healthcare team to develop individualized care plans while supporting residents and their families with empathy and professionalism.

Qualifications

• Graduate of an accredited Licensed Practical Nurse (LPN) program.
• Active LPN license.
• Experience in healthcare or long-term care preferred but not required.

Responsibilities

• Administer medications as prescribed and monitor for side effects.
• Monitor vital signs and report changes to the care team.
• Assist RNs and physicians with procedures and dressing changes.
• Document health status and care provided in medical records.
• Provide compassionate, patient-centered care and support to residents.
