Job summary

Vicar's Landing Oak Bridge Memory Care in Ponte Vedra Beach, FL is seeking an LPN to deliver compassionate, person-centered nursing care for residents with dementia. You will work closely with CNAs, RNs, and interdisciplinary teams to support clinical wellness and quality of life in a Montessori-inspired memory care environment.

Responsibilities include administering medications per physician orders, maintaining documentation, and supervising CNAs while ensuring safety and dignity.

Qualifications

• Active Florida Licensed Practical Nurse (LPN) license.
• Current CPR/BLS certification.
• Strong clinical assessment and observational skills.
• Excellent verbal and written communication abilities.
• Ability to exercise sound nursing judgment while following established care plans.
• Strong organizational and documentation skills.
• Ability to effectively supervise and support CNAs.

Responsibilities

• Provide direct nursing care in accordance with physician orders, nursing standards, and individualized care plans.
• Administer medications and treatments safely and accurately while maintaining documentation.
• Monitor residents for changes in condition and promptly communicate concerns to the RN or Director of Nursing.
• Perform routine nursing procedures, including wound care, catheter care, specimen collection, and vital sign monitoring.
• Assist residents in maintaining the highest possible level of comfort, function, and quality of life.
• Provide guidance and direction to CNAs; assist with admissions, transfers, and discharges.
• Collaborate with nurses, physicians, therapists, and interdisciplinary team members to ensure continuity of care.
• Serve as a resource for team members and contribute to a positive, supportive work environment.
• Communicate effectively with families regarding resident care and changes in condition.
• Maintain accurate, complete, and timely clinical documentation in the EMR.
• Follow infection prevention procedures, safety standards, and emergency protocols.
