Job summary

Vicar's Landing Oak Bridge Memory Care in Ponte Vedra Beach, FL is seeking a dedicated LPN to provide compassionate, person‑centered care to memory‑impaired residents. You will work with CNAs, nurses, and an interdisciplinary team to support clinical wellness, dignity, and quality of life.

You will administer medications, monitor conditions, and contribute to a calm, therapeutic environment that respects resident independence and safety.

Qualifications

• Active Florida LPN license.
• Current CPR/BLS certification.
• Strong clinical assessment and observational skills.
• Excellent verbal and written communication abilities.
• Ability to supervise and support CNAs.

Responsibilities

• Provide direct nursing care per physician orders and individualized care plans.
• Administer medications and treatments with proper documentation.
• Monitor residents for changes and promptly communicate to the RN or DON.
• Perform routine nursing procedures including wound care and catheter care.
• Assist residents in maintaining comfort, function, and quality of life.
• Guide CNAs and assist with admissions, transfers, and discharges.
• Collaborate with nurses, physicians, therapists, and the care team.
