Job summary

Vicars Landing Oak Bridge Memory Care seeks a compassionate LPN to provide resident-centered care in our Montessori-Inspired Memory Care community in Ponte Vedra Beach, FL. You will work with CNAs, RNs, and other clinicians to support residents living with dementia and maintain dignity and independence.

The role emphasizes collaboration, documentation, and delivering high-quality care in a memory-focused setting with ongoing professional development opportunities.

Qualifications

• Active Florida LPN license and CPR/BLS certification.
• Strong clinical nursing and assessment abilities.

Responsibilities

• Provide direct nursing care per physician orders and care plans.
• Administer medications and treatments safely and accurately.
• Monitor residents and report changes to RN or DON promptly.
• Perform wound, catheter care, specimen collection, and vitals checks.
• Support residents’ comfort, safety, and independence.
• Lead and supervise CNAs; participate in admissions, transfers, discharges.
• Maintain accurate clinical documentation in EMR; ensure safety compliance.

About This Role

As a Licensed Practical Nurse (LPN), you'll provide compassionate, resident-centered care within our Montessori-Inspired Lifestyle Memory Care Community, supporting residents living with Alzheimer's disease and other forms of dementia. You'll work closely with residents, families, CNAs, and healthcare professionals to create a safe, engaging, and supportive environment where residents can maintain their independence, confidence, and sense of self. What You'll Do

Resident Care & Clinical Support

• Provide direct nursing care in accordance with physician orders, nursing standards, and individualized care plans
• Administer medications and treatments safely and accurately
• Monitor residents for changes in condition and communicate observations promptly to the RN or Director of Nursing
• Perform routine nursing procedures, including wound care, catheter care, specimen collection, and monitoring vital signs
• Support residents in maintaining their highest level of comfort, safety, and independence

Montessori-Inspired Memory Care

• Support a person-centered approach that recognizes each resident's strengths, preferences, and abilities
• Encourage engagement through meaningful daily interactions and activities
• Help create a calm, supportive, and purposeful environment for residents living with memory impairments
• Promote dignity, choice, and quality of life in every aspect of care

Team Collaboration & Leadership

• Provide guidance and direction to Certified Nursing Assistants (CNAs)
• Collaborate with nurses, physicians, therapists, and interdisciplinary team members to ensure continuity of care
• Participate in admissions, transfers, and discharges as needed
• Communicate effectively with families regarding resident needs and changes in condition
• Serve as a resource and support for caregivers throughout your shift

Quality, Safety & Compliance

• Maintain accurate, timely, and complete clinical documentation within the electronic medical record
• Ensure compliance with infection prevention protocols, safety standards, and regulatory requirements
• Support resident rights, privacy, confidentiality, and dignity
• Participate in training programs, emergency preparedness drills, and continuing education opportunities
• Help maintain adequate supplies and equipment within the community

QualificationsMust Have Skills

• Familiarity with electronic medical records
• Knowledge of person-centered or Montessori-based care approaches
• Previous leadership or charge nurse experience
