Job summary
Vicar's Landing Oak Bridge Memory Care in Ponte Vedra Beach, FL is seeking an LPN to join a Montessori-inspired memory care community. You will deliver resident-centered nursing, follow physician orders, administer meds, and collaborate with CNAs, nurses, and physicians to support safety, independence, and dignity.
This role emphasizes compassionate care, strong documentation in EMR, and coordination with families.
Qualifications
- Active Florida LPN license (or equivalent) and CPR/BLS certification.
- Strong clinical nursing and assessment abilities.
- Excellent communication and interpersonal skills.
- Ability to supervise and support CNA team members.
- Strong organizational and documentation skills.
- Experience collaborating with interdisciplinary healthcare teams.
Responsibilities
- Provide direct nursing care per physician orders and individualized care plans.
- Administer medications and treatments safely and accurately.
- Monitor residents for changes in condition and report observations to RN/Director of Nursing.
- Perform routine nursing procedures and monitor vital signs.
- Support residents' independence, safety, and comfort.
