Beth Israel Lahey Health's Herrick House seeks an LPN to oversee medication management and coordinate residents' medical needs with physicians, the Director of Nursing, and families. The role requires documenting changes, supervising staff, and ensuring safe, compassionate care.

The position emphasizes collaborative relationships, ongoing health teaching, and adherence to regulatory guidelines, with a base hourly wage and influenza vaccination requirement.

Qualifications

• Graduate of a state approved and/or accredited School of Nursing.
• A minimum of 1 year of medical/surgical nursing or nursing in a skilled nursing facility is required.
• Current license to practice nursing from the Massachusetts Board of Registration. BLS certification.

Responsabilités

• Assess an individual’s basic health status, records and related health data.
• Participate in analyzing and interpreting recorded data to determine nursing care elements.
• Participate in planning and implementing nursing interventions within care plans.
• Incorporate the prescribed medical regimen in the nursing plan of care.
• Provide health teaching to patient and family to maintain optimal health.
• Evaluate outcomes of nursing interventions and adjust plans as needed.
• Provide safe patient care and coordinate delivery of quality care.
• Develop a culturally competent plan of care respecting patient values.
• Deliver care appropriate to the patient’s age.
• Establish collaborative relationships with nurses, physicians, patients and families.
• Educate patient and family about illness and treatment including prevention.
• Support community-based activities within the Lahey community.
• Demonstrate clinical leadership and compliance with policies.
• Demonstrate the role of LPN as a teacher and readiness for regulatory requirements.
