Odyssey House Louisiana, Inc. seeks an LPN to support residents in the Adult Residential Program. You will orient clients, participate in medical care coordination with the Medical Staff, and assist in managing residents’ medications and healthcare needs.

Your duties include ongoing health education, monitoring treatment plans, and documenting health information. The role requires independent work, adherence to HIPAA, and collaboration with CHC staff.

Qualificações

• LPN license required.
• CPR certification required.
• Experience with medication management for multiple residents.
• Experience in a residential treatment setting preferred.

Responsabilidades

• Assess potential residents for program eligibility.
• Complete updated assessments post-admission.
• Coordinate medication stock, refills, and administration.
• Document health information in resident charts per standards.
• Attend treatment team meetings and communicate care plans.
• Discharge planning and ensuring medication continuity.
