Job summary

CHR Consulting Services, Inc. is seeking an LPN to deliver nursing care in a resident-focused unit. You will administer medications, document treatments, and monitor patient status to ensure safety and quality care across shifts.

The role requires strong communication, basic computer skills, and the ability to lift up to 20 pounds. Experience with PCC e-MAR is preferred. Kentucky-based facility with a collaborative team environment.

Qualifications

• Licensed practical nurse (LPN)
• Strong written and verbal communication skills; able to follow written and verbal instructions.
• Basic computer knowledge; PCC e-MAR experience preferred.
• Physical endurance; ability to lift a minimum of 20 pounds.

Responsibilities

• Ensures the care and safety of all residents in the nursing unit
• Ensures that individual care plans are followed
• Administers and documents medications, treatments, and other nursing services
• Makes a count of controlled substances on the assigned nursing unit
• Records current condition of assigned resident
• Receives and gives an accurate report of the resident condition at shift change
• Communicates to the nursing team and medical provider as indicated and required.
• Assists in the orientation of new staff
• Maintains competency in all the tasks needed by CMS
