Saint Dominic's Family Services is seeking a Registered Licensed Practical Nurse (LPN) to provide comprehensive nursing care in a residential setting. The LPN administers medications, monitors health conditions, documents care, and collaborates with residential and clinical teams to promote residents’ health and well-being.

The role requires current NY/LPN licensure, BLS certification, and preferably 1 year of OPWDD-certified residential experience.

Qualifications

Current New York State Licensed Practical Nurse (LPN) license in good standing.
Ability to provide nursing services in accordance with the LPN scope of practice.
1 year experience working in an OPWDD-certified residential program or similar community-based setting preferred
Current Basic Life Support (BLS) certification from a recognized provider, maintained throughout employment.

Responsabilités

Provide nursing care to individuals in accordance with their assessed medical needs and nursing plans.
Monitor health condition changes and report them to supervisors or medical staff.
Perform nursing evaluations and follow established health protocols.
Administer medications and treatments per physician orders and protocols.
Coordinate medical appointments and follow-up with care teams.
Maintain accurate medical documentation and EMR records.
