Five Star Nursing in Allentown, PA is seeking a compassionate Licensed Practical Nurse (LPN) to join our long-term care and rehabilitation team. The LPN will deliver high-quality resident-centered nursing care and collaborate with RNs, physicians, therapists, and staff to support care plans.

You will monitor residents, administer medications, document care, participate in care planning, and supervise CNAs while upholding safety and regulatory standards.

Qualifikationen

• Must hold an active LPN license in the state of employment.
• Minimum 2 years nursing experience in skilled nursing, rehabilitation, long-term care, or hospital.
• Proficient in medication administration and patient assessments.
• Strong communication and teamwork abilities.
• Ability to work independently and within a multidisciplinary team.

• Provide direct nursing care to residents, respecting dignity and promoting comfort.
• Administer prescribed medications and treatments according to protocols.
• Monitor vital signs and document changes to ensure timely care.

• Maintain infection control, safety policies, and regulatory requirements.
• Ensure complete medical records and electronic documentation.
• Participate in care planning meetings and quality improvement initiatives.
• Respond to resident emergencies to ensure safety and well-being.
• Promote dignity, respect, and quality of life for residents.
