Qualifications

• Current and active LPN license in the state of employment.
• Minimum of 2 years of relevant nursing experience in a skilled nursing facility, rehabilitation center, long-term care, assisted living, or hospital setting.
• Proficiency in medication administration, nursing procedures, and patient assessments.
• Strong critical thinking and clinical judgment skills.
• Excellent communication and interpersonal abilities.
• Capable of working independently and as part of a multidisciplinary team.

Responsibilities

• Provide direct nursing care to residents and patients, respecting dignity and comfort.
• Administer prescribed medications and treatments following facility protocols and physician instructions.
• Monitor and document residents' vital signs and responses to treatments.
• Conduct assessments of residents' physical, mental, and emotional status; report changes promptly.
• Assist with admission, transfer, and discharge processes in line with regulatory standards.
• Develop relationships with residents, families, and healthcare team to foster a supportive environment.
• Supervise and support CNAs in delivering quality resident care.
• Maintain infection control, safety policies, and regulatory compliance.
• Ensure complete and accurate medical records and electronic documentation.
• Participate in care planning meetings and quality improvement initiatives.
• Respond swiftly to resident emergencies and changes in condition.
• Promote dignity, respect, and enhanced quality of life for all residents.
