Job summary

Trinity Homes in Minot, ND is seeking a dedicated Long Term Care LPN to provide direct nursing care under RN supervision. You will support individualized care plans to promote comfort, safety, dignity, and quality of life for residents.

This full-time role involves collaboration with CNAs, providers, and the care team to ensure continuity across shifts, accurate documentation, and compassionate engagement within a safe, therapeutic environment at Trinity Homes.

Qualifications

• Current or temporary LPN license in ND or compact LPN licensure.
• BLS certification is required within the orientation period.
• LPN program/licensure evidenced by licensure.
• Nursing home experience preferred, but not required.

Responsibilities

• Assist the RN with assessments, data collection, and continuous monitoring of residents’ status.
• Provide delegated nursing care, including medication administration, treatments, wound care, and chronic condition monitoring.
• Document all assessments, interventions, and resident responses in the electronic record for regulatory compliance and continuity of care.
• Communicate changes in resident condition to the RN, provider, and family and assist with care plan updates.
• Supervise CNA staff during daily care activities, ensuring safety and resident dignity.
• Maintain safe, clean, homelike environment by following infection prevention and safety procedures.
