Job summary

Reliant Care Management Company is seeking a compassionate LPN or RN for PRN night shifts to provide high-quality nursing care to residents. The role includes conducting assessments, following protocols, and documenting changes, along with admission and discharge duties.

You will oversee CNAs and CMTs, ensuring adherence to facility policies and responding promptly to emergencies. Valid LPN/RN licensure is required.

Qualifications

• LPN or RN licensure or certification required.

Responsibilities

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings.
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications.
• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation.
• Provide wound care: follow protocols, irrigate, pack, and remove dressings.
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies.
• Respond effectively to emergency situations.
