Hall of Fame Healthcare Center in Canton, OH seeks an LPN with empathy and patience to care for residents. This full-time position covers 7am–7pm on day shifts, with responsibilities including assessments, admissions, discharges, wound care, and supervising CNAs. LPN license required and strong teamwork is essential.

Qualificações

• Compassionate caregiver able to assess and document changes.
• Capable of patient-focused care with strong communication.
• Experience guiding CNAs and coordinating care with the team.
• Proficient with electronic medical records and MAR/TAR processes.

Responsabilidades

• Conduct assessments, follow protocol, report changes, document findings.
• Perform admission duties: assessment, MAR, TAR, POS, orders, medications.
• Manage discharges: documentation, notify physician/guardian, arrange transport.
• Provide wound care following protocols and dressing changes.
• Lead CNAs and CMTs, ensuring compliance with policies.
• Respond to emergencies promptly.
