Job summary

You will lead CNAs and CMTs, handle admissions, documentation, wound care, and respond to emergencies, all within a culture built on integrity, work ethic, and clinical excellence.

Qualifications

• LPN license in good standing.
• Ability to perform assessments and document findings.
• Team leadership and collaboration with CNAs/CMTs.

Responsibilities

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications
• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation
• Provide wound care: follow protocols, irrigate, pack, and remove dressings
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies
• Respond effectively to emergency situations
