Reliant Care Management Company in Missouri is seeking a compassionate Licensed Practical Nurse (LPN) to support residents' daily care, safety, and comfort with empathy and professionalism.

You will conduct assessments, complete admissions, manage discharges, provide wound care, and oversee CNAs and CMTs while following facility protocols and documenting changes. This on-site role offers comprehensive benefits and a collaborative team environment to deliver high-quality patient care.

Qualifications

• Must hold an LPN licensure/certification.

Responsibilities

• Conduct assessments, report changes, document findings
• Perform admission duties: MAR, TAR, POS, physician orders, and order medications
• Manage discharges: documentation, notify physician/guardian, arrange transportation
• Provide wound care per protocols including irrigation, packing and dressing changes
• Lead CNAs and CMTs, ensuring compliance with facility policies
• Respond effectively to emergencies
