Job summary

Genesis Home Care is seeking a Licensed Practical Nurse (LPN) I to provide skilled nursing care in patients’ homes. You will educate patients and families, document care, and ensure compliance with Medicare guidelines and state regulations.

Qualifications

Minimum of one year of experience as a LPN in an acute care setting is required; two years is preferred.
Home care experience is desired.
Ability to effectively communicate with patients, families, physicians and co-workers.
Ability to remain calm, be compassionate and patient.
Knowledgeable on nursing best practices.
Ability to make appropriate nursing judgments.
Follow directions and work as a team member.

Responsibilities

Provides skilled nursing care in a patient’s home as ordered by the attending physician.
Performs care by following order sets and goals made by RN.
Observes and monitors patient conditions.
Notifies RNCM of changes in patient condition.
Administers medication as prescribed by the physician.
Counsels patient and family on disease/injury processes and plan of care.
Documents all aspects of treatment and patient education.
