Job summary

Altra Home Health, LLC in Muncie seeks a compassionate Licensed Practical Nurse (LPN) to deliver in-home care to pediatric and adult patients, including complex needs like G-tubes, trach, or ventilator support. You will assess, treat, and educate, coordinating with physicians and therapists to ensure high-quality care in the patient’s home.

Qualifications

• Active LPN license in Indiana or a compact LPN license.
• 1–2 years of clinical nursing experience; home health and pediatric preferred.
• Current CPR certification.
• Experience with G-tube, trach, or vent care is a plus (training provided).

Responsibilities

• Conduct comprehensive head-to-toe assessments and care plan implementation.
• Deliver hands-on care including G-tube feedings, medications, wounds, trach and ventilator care.
• Provide professional, empathetic patient interactions.
• Educate patients and caregivers on disease management and emergency response.
• Collaborate with physicians, therapists, and team members to ensure quality care.
• Maintain accurate, timely documentation per agency and state standards.
• Uphold safety, infection control, and patient privacy standards.
