Job summary

Altra Home Health is seeking a compassionate LPN to join our home health team serving pediatric and adult patients in their homes. You will perform comprehensive assessments, follow personalized care plans, administer medications, and provide skilled nursing care including G-tube, trach, and ventilator support as required.

Qualifications

• Active LPN license in Indiana or a Compact license.
• 1–2 years of clinical nursing experience in home health; pediatric experience preferred.
• Current CPR certification.

Responsibilities

• Perform head-to-toe assessments and implement individualized care plans.
• Provide hands-on care including G-tube feedings, medications, wound care, trach care, and ventilator support as needed.
• Maintain professional, empathetic patient interactions and educate patients and caregivers.
