Job Summary

Altra Home Health is looking for a caring and skilled Licensed Practical Nurse (LPN) to become part of our team. In this rewarding position, you'll deliver personalized, top-notch care to pediatric and adult patients in the comfort of their own homes. Many of our patients face complex medical challenges, including gastrostomy tubes (G-tubes), cerebral palsy, chronic conditions, and occasionally tracheostomy or ventilator support. You'll be instrumental in fostering safety, comfort, and quality of life- empowering patients to receive the care they need right where they feel most comfortable: home.

Responsibilities

• Conduct comprehensive head-to-toe assessments and carry out personalized care plans.
• Deliver hands-on care such as G-tube feedings, medication administration, wound care, trach care, and ventilator support.
• Approach every patient interaction with professionalism and genuine empathy.
• Guide and empower patients and caregivers with education on disease management, emergency response, and therapeutic techniques.
• Partner closely with physicians, therapists, and fellow team members to deliver the best possible care.
• Keep accurate, timely documentation of all care activities in line with agency and state standards.
• Uphold the highest standards of safety, infection control, and patient privacy.

Shift Information

• Shifts range from 4 to 12 hours, tailored to patient needs and nurse availability.
• Flexible scheduling for day, evening, and weekend shifts.
• Choose from consistent long-term assignments or varied schedules.

Qualifications

• Active LPN license in Indiana or an active compact LPN license.
• Minimum of [1-2] years of clinical nursing experience (home health and pediatric experience strongly preferred).
