Job summary

Altra Home Health is seeking a licensed practical nurse (LPN) to deliver in-home care for pediatric and adult patients in Rochester, Indiana. You will perform assessments, administer medications, and collaborate with a multidisciplinary team to promote safety and quality of life at home.

Shifts range from 4 to 12 hours with flexible scheduling; Indiana LPN license and CPR certification required. Training is provided for G-tube, trach, and ventilator care to support our medically complex patients.

Qualifications

• Active LPN license in Indiana or a compact license.
• Minimum of 1–2 years of clinical nursing experience, home health and pediatric experience preferred.
• CPR certification current.
• Ability to work independently with strong patient advocacy.

Responsibilities

• Conduct head-to-toe assessments and implement personalized care plans.
• Provide hands-on care including G-tube feedings, medications, wound care, trach care, and ventilator support.
• Maintain professionalism and empathy in every patient interaction.
• Educate patients and caregivers on disease management and emergency response.
• Collaborate with physicians, therapists, and teammates to ensure quality care.
• Document all care activities accurately and timely per state and agency standards.
• Uphold safety, infection control, and patient privacy standards.
