Job summary

FutureCare in Baltimore, MD is seeking a Licensed Practical Nurse to join our care team on site. Under RN supervision, you will assist with care planning, treatments, and resident assessments while delivering compassionate, quality care.

The ideal candidate holds an active Maryland LPN license, CPR certification, and at least 1 year of nursing experience in long-term or acute care; strong teamwork and communication are essential.

Qualifications

• Active LPN license in Maryland.
• 1 year of nursing experience preferred.
• CPR certification required.

Responsibilities

• Under the direct supervision of the RN, assist in care planning and treatments for residents and consults with other nursing staff as appropriate with applying knowledge and resources
• Demonstrate basic knowledge and skills required to care for the resident population on an assigned unit
• Review resident medical records to ensure nursing practice provided is within State and Federal guidelines
• Document care provided and report changes in resident status to shift supervisor
• Review all referrals for consultation, treatment or resolution of problems are valid and applicable to the documented nurse plans
• Seeks opportunities for professional growth and expansion of responsibilities to maintain competency levels
• Set and implement goals and policies consistent with the philosophy and purpose of FutureCare’s nursing facility
