Job summary

Avamere is seeking an Licensed Practical Nurse (LPN) for the Rogue Valley location in Medford, OR. This full-time role supports residents under RN supervision, performing vital signs, medication administration, wound care, and daily living activities.

Responsibilities include documenting patient history, monitoring changes, and coordinating with CNAs and the health care team to ensure quality care. CPR/BLS certification is required, with opportunities in long-term care settings.

Qualifications

• Must have a nursing degree from an accredited college or university.
• Must possess a current, unencumbered LPN license to practice in Oregon.
• CPR/BLS certification required during employment.

Responsibilities

• Provide basic nursing care including vital signs, wound care, and hygiene.
• Administer medications as prescribed under RN supervision.
• Record patient history and monitor changes in condition.
