Job summary

Avamere Rogue Valley in Medford, OR is seeking a Licensed Practical Nurse to provide basic nursing care, monitor vital signs, administer medications, and assist residents with daily activities under RN supervision. You will ensure hygiene, mobility, nutrition, documentation, and effective communication with residents, families, and the healthcare team, contributing to a supportive, safe care environment.

A current LPN license, CPR/BLS, and experience in long-term care preferred; Avamere offers

Qualifications

• Nursing degree from an accredited college or university.
• Active LPN license in the state.
• CPR/BLS certification required during employment.
• Experience in long-term care or geriatric settings preferred.
• Experience with electronic medical records documentation.

Responsibilities

• Provide basic nursing care and monitor residents.
• Take and record vital signs accurately.
• Administer medications as prescribed under RN supervision.
• Perform wound care and monitor infections.
• Assist with hygiene, mobility, and nutrition.
• Document care and changes in resident condition.
• Communicate with residents, families, and healthcare staff.
• Work under RN supervision and supervise CNAs.
• Maintain resident safety and infection control.
